A coroners Report,

Hey all, its time to finally enter something on my blog as this is the very first one i would love to here good/or bad criticism please don't be afraid as i don't bite yet.
For awhile i was strolling through some government reports a week ago and came across, a great report, i feel after i read the whole thing (150 pages)that a lot of it has to do with the courses we are doing

This is a copy and official report of an investigation into a death of Ian Ward on 27th Jan 2008 at Kalgoorlie district hospital.
Even though this was not an accident the coroner Mr Alister Neil Hope made quite a few recommendations after the death due to the number of facts relating to the death,many what caught my eye especially when you read all about the vehicle, and all the communications.

    Thank you for sharing the report; I will have a look at it ... when you read it .. what questions did it raise for you? Did you note any multiple perspectives where people looked at the same situation but read it differently? What were the key lessons that we could take from it?
